Instructions

An SF-182 is required for some programs. If your program identifies the need for this, please complete it as follows:  

1. Section A: Complete all fields EXCEPT SSN and DOB (Field 2 and 3). **IMPORTANT: We cannot accept a form with a SSN or DOB**
2. Section B: Complete Course Title (Field 2a), Training Start Date (Field 3), and Training End Date (Field 4). The remaining fields may be left blank.
3. Section C: Leave this section blank.
4. Section D: A Supervisor signature is required in Field 1a. Please follow your agency guidelines regarding additional signatures.
5. Send the completed SF-182 to 4thEstateDACM@dau.edu.
